2017 Polar Plunge

Family Health Medical Services is proud to announce we held our 3nd Annual Polar Plunge, on Saturday, February 18, 2017, at Mayville Lakeside Park during the President’s Day Weekend Festival. We are looking forward to the 4th Annual in February 2018.

Giving back to our community is a principle value at FHMS. We take great pride in our commitment to support organizations that directly benefit the quality of life in our community. This year we will partner with The Food Bank of WNY, whose mission is to obtain nutritious food and support from public and private sources and efficiently distribute these resources to the hungry in Western New York through there member agencies. Proceeds from this year’s polar plunge will allow them to support their member agencies here in Chautauqua County in the Fight Against Hunger.

We wish to appeal to local business’ and community members to join us in efforts to help fight hunger here in Chautauqua County. Join us by participating in this crazy, fun event or by sponsoring the cause. Those who provide sponsorship will have high visibility with identification on t-shirts, posters, and media publicity, as well as the opportunity to have banners and promotion on the day of the event.

We Are a Certified Patient-Centered Medical Home

What is Patient-Centered Medical Home ??

NCQA Logo

A patient-centered medical home is a model of care that strengthens the clinician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship. Each patient has a relationship with a primary care clinician who leads a team that takes collective responsibility for patient care, providing for the patient’s health care needs and arranging for appropriate care with other qualified clinicians. The medical home is intended to result in more personalized, coordinated, effective and efficient care.

A medical home achieves these goals through a high level of accessibility, providing excellent communication among patients, clinicians and staff and taking full advantage of the latest information technology to prescribe, communicate, track test results, obtain clinical support information and monitor performance.
